Each practitioner reviews your chart, writes a prescription, and sends it electronically to a pharmacy. On paper, the process looks seamless. In practice, it can be quietly dangerous.

The Clinical Stakes of Duplicate Prescriptions

Duplicate prescriptions are not merely a matter of administrative inconvenience or wasted medication costs. They carry genuine clinical risk. Therapeutic duplication—in which two drugs from the same pharmacological class are prescribed simultaneously—can amplify side effects, increase the likelihood of adverse drug events, and in some cases produce life-threatening outcomes.

Consider a patient managing both anxiety and a sleep disorder who is seeing a psychiatrist and a primary care physician independently. What Pharmacy Systems Can and Cannot Do

Modern pharmacy dispensing software includes drug utilization review (DUR) tools designed to flag potential interactions, contraindications, and therapeutic duplications. These tools are genuinely valuable—but they are only as effective as the data they can access.

When a patient fills all of their prescriptions at a single pharmacy, the pharmacist has a reasonably complete picture of their medication profile. The DUR system can cross-reference new prescriptions against existing ones and alert the pharmacist to potential concerns. Pharmacists and prescribers are generally required to consult these databases before dispensing or prescribing opioids, stimulants, and other scheduled medications. But PDMPs are not universally applied to non-controlled medications, and their effectiveness depends on consistent, timely data entry by all dispensing entities—a standard that is not always met.

Patients who have experienced the consequences of undetected duplicate prescriptions often describe a similar sequence of events. They notice unexpected side effects, feel unusually fatigued or cognitively impaired, or experience a new symptom that prompts an additional medical visit. In many cases, the connection between their symptoms and a medication overlap is not identified until a pharmacist, hospitalist, or clinical pharmacologist conducts a comprehensive medication review.

In other instances, the financial impact surfaces first. A patient may notice that their insurance has denied a claim for a medication because a therapeutically equivalent drug was already dispensed within the same coverage period. This kind of denial, while frustrating, can serve as an inadvertent alert that something in the prescribing chain has gone uncoordinated.

Telehealth visits, by their nature, often occur outside the patient's established care network.
